PASSPORT SERVICES

Passport services is a branch of the Bureau of Consular Affairs within the US Department of State. We here at the Carroll County Superior Court Passport Acceptance Agency are responsible for accepting passport applications from eligible US citizens, while ensuring the integrity of the US passport, and providing the information and services by which Americans can obtain, replace, or request a correction to a previously issued valid passport.  A US passport is an identity and citizenship document, allowing US citizens to travel internationally. 


We are open 5 days a week.  We accept appointments and on occasion we take walk-ins when time permits and provided all paperwork is in order.  Our first available appointment each day is when the courthouse opens at 8:30am.  The last available appointment is at 4:00pm in order to allow time for processing applications.  Please call us any time for an appointment at (770) 830-5380.  Be patient as our agents are quite busy assisting other applicants with their allotted appointment times.  Leave a detailed message and our dedicated agents will return your call as soon as possible.  Do NOT leave multiple messages as this will delay our response time.

Instructions for applying for a NEW PASSPORT:  BRING ALL OF THE FOLLOWING WITH YOU TO YOUR APPOINTMENT!


1 - New Passport Application completely filled out for each person   -   DS-11 New Passport Form


2 - Certified Copy of each person's Birth Certificate (long form)


3 - Current Valid Driver's License or State ID


4 - A check or Money Order (for the exact amount) made out for $130 for each adult 16 and over and $100 for each minor child 15 and under to the US DEPARTMENT OF STATE.  For a family, one consolidated check made out for the entire amount will suffice.


5 - A check or Money Order made out for $35 for each person to the CLERK OF COURT for the processing fee.  The photos here are $10. If the photos are made onsite, the check will be made out for $45 instead. 


6 - Bring your most current or expired passport - if any.  If you have lost your most recent passport, you will be required to fill out the DS-64 LOST PASSPORT FORM and bring it with you when you apply for a new passport.


Photos may be obtained from other places in town that offer passport photo services.  As these places charge more for their passport photos, understand that the US DEPARTMENT OF STATE requires strict protocols when it comes to the photos that are used in the passports that accompany your application.  We are here to ensure your passport application has the best possible chance of navigating the processing procedure so you will be issued a new passport. We will make certain that the photos you bring in match the strict guidelines the USDOS have set.  We do offer on-site photos for only $10.00 so you can be guaranteed your passport photo is accurate.



Important Information:


For children under 16, both parents MUST be present at the time of the appointment.  If both parents are unable to be present, the DS-3053 STATEMENT OF CONSENT FORM must be presented in the place of the absent parent's signature.  A front and back copy of their Valid Driver's License MUST accompany this form.  If this form is not included with the Passport application when the second parent is absent, the passport application will not be accepted.
