Passport Services is a directorate of the Bureau of Consular Affairs within the US Department of State.  We, as your local Carroll County Passport Acceptance Agency, are responsible for accepting passports to eligible US citizens, ensuring the integrity of the US passport, and providing the information and services by which American can obtain, replace, or request a correction to a previously issued valid passport. A US passport is an identity and citizenship document, allowing US citizen to travel internationally.  We welcome you to the Carroll County Clerk of Court's Office to apply for your passport and mark that off your pre-travel list. 

                     Below is the information needed for your new passport appointment: 

1. An appointment is needed: please call (770) 830-5830 x1211 for an appointment

2. If applying for a new passport, you will need to use the DS-11 passport form, completely filled out prior to arrival.

a. Certified copy of your Birth Certificate.

b. Colored photo (our office will make photos on site for $10.00 - cheapest in town)

c. ID (Current Driver's License or State ID)

3. Money order or Check made out to the US Department of State for the cost of the passport application (fee chart link below)

4. Money order or Check made out to the Clerk of Court for processing fee and/or photo for $35/$45 (fee chart link below)
